Summary. The SuperPatch Wellness Assessment is a screening tool, not a medical diagnosis. When you take the assessment, we collect contact details, demographics, and questionnaire answers to generate your results, store your session, email your insights via Klaviyo (if you consent), and add you to our marketing list (if you consent). We keep assessment session data for up to 24 months, never sell it, and you can ask to access or delete it at any time.

Who we are

This service is operated by SuperPatch ("we", "us"). We are responsible for the personal information handled through this consumer wellness assessment and act in line with applicable privacy laws, including Canada's Personal Information Protection and Electronic Documents Act (PIPEDA) where applicable.

What we collect

Your first name and email address.
Demographics you provide (age range, sex assigned at birth, and gender identity).
Your questionnaire answers, including wellness-related responses, and the scores derived from them.
A record of your consent: policy and terms versions, date/time, and IP address at the moment you agreed.

How we use it

We do not sell your information. Super Patches are optional, drug-free complementary wellness tools — not treatments or diagnoses.

To generate your wellness results and lifestyle suggestions.
To email your insights through Klaviyo when you consent to receive your report.
To add you to SuperPatch email updates when you consent to marketing.
To show optional Super Patch wellness-product suggestions related to your focus areas.

Who we share it with

We use service providers that process data on our behalf:

They may only use your information to provide those services to us.

Cloudflare — hosts the application and stores consented session data in Workers KV.
Klaviyo — stores your email profile (including session UUID and demographics you provided) and sends consented marketing and results emails.
PostHog — product analytics. After you save results with your email, we identify that session so we can understand how the assessment is used.

How long we keep it

Assessment session records in Cloudflare KV are retained for up to 24 months (automatic expiry), then removed, unless a longer period is required by law. Klaviyo profiles remain until you unsubscribe or request deletion.

Your rights

You may request access to or deletion of your assessment data by contacting us using the email address you provided at intake (or via the contact channel listed on SuperPatch.com). We will verify your request and respond within a reasonable time, including deleting the KV session and suppressing/deleting the related Klaviyo profile where applicable.
